Case Management Services

All of the youth that come through The Good Seed’s programs either through the emergency shelter, family crisis housing, or transitional housing are given access to case management services to assist them in accomplishing their specific goals and needs. The Good Seed wants these youths to not only survive, but thrive when they transition into new environments. Supportive services by caring staff and peers are the backbone of The Good Seed’s mission and organizational culture.

Current case management practices within The Good Seed’s shelter programs are constantly evolving to fit the needs of the young people we serve. Crisis management is an immediate service that takes place when a new youth enters our shelter doors. Good Seed’s staff members are comprehensively trained in the usage of Psychological First Aid and Trauma-Informed care to support clients that are experiencing distress. The Good Seed also utilizes the practice of motivational interviewing when supporting youth in process determining priorities, and their life goals. Our desire is to engage the intrinsic motivation within each young person to have them begin an internal process of self-reflection and discovery. Good Seed’s staff understand that a human connection in a non-intrusive and compassionate manner is necessary to plant the seeds of trust. And, that before we can water them with supportive services and resources to help them take control of their lives, the soil of their mind and hearts have to be cared to first.

Intensive Case Management

The ICMS program assists high and medium acuity individuals ages 18-80 with a mental or physical disability, to obtain and maintain housing, and reduce their reliance on the emergency room as their primary method of health care, through individualized planning services.

ASCENT Program: Reentry Intensive Case Management Services (RE-ICMS) Empowering Formerly Incarcerated Individuals to Thrive

The ASCENT Program provides trauma-informed, dignity-centered case management to support adults transitioning from incarceration back into community life. As part of our comprehensive suite of services, ASCENT is dedicated to helping individuals overcome barriers and build a stable, fulfilling future.

Who We Serve

  • Adults recently released from jail, prison, or detention
  • Individuals on probation, parole, or post-release supervision
  • Those with co-occurring mental health or substance use challenges
  • High-risk individuals with histories of homelessness or justice involvement

  • Assist with resume building, job readiness, and placement

  • Offer financial literacy and budgeting guidance

  • Advocate for record expungement or legal aid

Social Reintegration

  • Facilitate family reunification and community engagement

Our Approach

Our trauma-informed model emphasizes safety, choice, and empowerment. ASCENT’s dedicated team includes:

  • Intensive Case Managers with low caseloads (1:15–1:30) for personalized care
  • Peer Support Specialists with lived experience to foster trust
  • Multidisciplinary Teams coordinating housing, health, legal, and workforce services

ECM Karen Healthcare

ECM is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical need of Members with the most complex medical and social needs.  ECM provides systematic coordination of services and comprehensive care management that is community based, interdisciplinary, high touch and person centered.

 

The program aims to coordinate all care for members who receive it, including across the physical and behavioral health delivery systems. ECM is a statewide Medi-Cal benefit that focuses on Improving care coordination, Integrating services, Addressing SDOH, improving health outcomes, decreasing ER and in patient hospital visit and facilitating community resources. ECM is designed to ensure all Medi-cal managed care members have access to comprehensive set of services based on their needs and preferences, across the continuum of care and with the highest needs.ECM provides services to 9 populations of focus.

Drop-in Center Allen Community/Healthcare

The Good Seed Drop-In Center (DIC) offers a safe, stable, and supportive environment aimed at promoting the self-sufficiency and well-being of Transitional Age Youth (TAY) aged 18-24. Our comprehensive, low-barrier support model addresses immediate needs while guiding clients toward long-term independence.

To promote dignity and stability, the DIC provides critical daily resources, including daily hot meals, access to personal hygiene facilities (showers), and on-site laundry services for clients.

Specialized Services

We offer a full spectrum of targeted support designed to address key barriers TAY face:

  • Housing Navigation: Comprehensive assistance with placement, enrollment, and navigation through various housing programs.
  • Mental Health: Direct access to mental health support services facilitated through partnerships with the Department of Mental Health (DMH).
  • Workforce Development: Robust job readiness training and workforce development programs to equip clients with marketable skills for employment.
  • Educational and Legal Assistance: Active assistance with pursuing further formal education, including enrollment in training programs, trade schools, colleges, and universities. We also provide support for obtaining and formalizing critical legal documents necessary for employment and housing.
